Extended Aged Care in the Home (EACH)

The Commonwealth’s Extended Aged Care in the Home, or ‘EACH’ programs support those with high level, complex care needs, equivalent to nursing home care. Services are flexible and tailored and include those offered under Community Aged Care Packages (CACP), but also extend to higher level services of medical and allied health support including continence, wound and medication management as well as high level nursing support.

Our Program Coordinator will meet with you to discuss your needs and eligibility for the service. EACH-D, or ‘Extended Aged Care in the Home - Dementia Support’ services are also available to those with an assessed dementia requiring additional support above and beyond an EACH program. A valid ACAT Assessment is also required for these packages.
