Illawarra Transitional Aged Care Services (ITAC)

The ITAC Program provides short term support (up to 12 weeks) for older people at the conclusion of a Hospital admission who require more time and support to rehabilitate at home. Our services work closely with the ITAC team and other care professionals to ensure the required level of care and support is offered within the home environment.
